U.S. High School Transcript Assessment Form
When you completed the CCCApply application, you provided De Anza with your U.S. high
school transcript information. Most students earn their English or ESL and math course

To view your placement, log in to MyPortal and open the Apps section, then click on the Student Registration tile. When the registration landing page opens, look under Student Records and click on View My Placement Results.
If you don't see a placement, you'll need to submit the U.S. High School Transcript Assessment Form to the Assessment Center. Click the button to download the form, then fill it out and email it to firstname.lastname@example.org
