Help Us Get Your Name Right

Sometimes a person's given name doesn't reflect who they are. Students may identify themselves by a name that is different from their legal name, or they may legally change their name, for a variety of important reasons. Here are some ways to help us get your name right.


Tell Us Your Preferred or True Name

You can indicate the true or preferred first name as you'd like it to appear on unofficial documents, such as class lists, grade reports and unofficial transcripts. To do this, use the

This form is also available on the Admissions Student Forms webpage and on MyPortal. (Open the Apps section in MyPortal > Click on the tile for Adobe Sign Student Forms > Look under Student Webforms > Select the De Anza True/Preferred Name Form.)


Tell Us If You Legally Change Your Name

You can also notify the college if you have legally changed your name as it appears on government and legal documents. This will enable us to change the name that appears on official transcripts, 1098-T tax forms and other official documents. To do this, use the

The information and forms on this webpage are intended for students. College employees who wish to update their name as it appears in college or district records should contact the Personnel Services unit in the district Human Resources office.
